Hey!
ST is not the best but this should easily get you to T15 maps. I will work on a stronger ST version + uniques soon! This is only posted due to demand from viewers :D
For gear,
Weapon: as much physical damage + meleee skills
Armour: as much fire resists + strength
For better ST sacrifice more Area of Effect for example and use 6 socket gems (I do not have this yet myself).
How to clear:
Activate 
Herald of Ash and start 
Leap Slam with Brink to cause the target to be "Primed for stun" followed by 
Boneshatter with Reach & Magnified Effect for INSANE clear - This should blow up the entire screen.
To finish of rares incase they do not die use 
Hammer of the Gods or 
Perfect Strike
