Potato Thrower IV you will need to kill an enemy known as Voraxis, which can be found in the Timid Kyle's Neglected Opening Abandoned Auger Mine within the Fadefields - After killing Voraxis, you can use Moxxi's Encore Machine to reset them and try again for another chance at the drop
