The expansion completely changed Blood Wave through
Hematolagnia Blood Wave, removing the cooldown entirely and turning it into a spammable Core Skill. Because of that, the old Blood Wave gameplay loop of collecting Blood Orbs and solving cooldown reduction is effectively gone. Instead, we focus on solving resource and scaling Overpower.

Build Variants
This is the intended version and what the guide footage is built around.
The major upgrades come from:
Heir of Perdition
Ring of Starless Skies
Banished Lord's Talisman
Heir gives us an enormous damage boost while also helping round out our stat requirements.
Starless solves multiple problems at once by reducing Blood Wave cost while also providing damage, attack speed and helping smooth out the build's resource economy.
Banished Lord's Talisman becomes one of our strongest multipliers because we're permanently stacking Overpower effects through Blood Wave.
Once assembled, this version becomes extremely simple to play. Most of your buttons become supportive while Blood Wave carries almost all of your damage.
This is the fastest, smoothest and strongest version.
Assigned Skills

We keep Skeletal Warriors active purely for utility.
They'll generate Blood Orbs, corpses, healing and help maintain Flesh-Eater uptime automatically. Since our rune setup also picks up Blood Orbs for us, this becomes an effortless source of sustain and damage.
Sacrifice Skeletal Mages and Golem.

Talismans

Charms are extremely strong this season and make up a surprisingly large portion of the build's damage.
Our highest priority is the unique Red Blessing, as the extra damage per Overpower stack scales incredibly well with Banished Lord's Talisman and ends up being one of our strongest multipliers.
For stats, prioritize:
- Ranks
- All Stats
- Blood Skills
- Resistances
For your Seal, get the extra Charm Slot first and pair it with Blood Binder 5-set.
The set gives us a massive damage increase while also adding valuable Max Life, making it the best overall setup for this build.
Skill Tree
0 (0)Most of the tree exists to support
Blood Wave rather than deal damage directly.
Reap is mainly there for early Essence generation, corpses and extra Blood Orbs.
Sever gives us mobility and becomes the reason the build feels so fast while farming.
Bone Prison provides attack speed, Vulnerable uptime and additional Essence generation for harder content.
Blood Mist gets converted into a mobility tool instead of a defensive button.
Iron Maiden and
Decrepify are automated through runes and mainly exist for executes, Blood Orbs, cooldown support and utility.
Everything ultimately feeds into Blood Wave, Hematolagnia and maintaining permanent Overpower scaling.
Paragon Board
If you're missing Paragon points, prioritize Legendary Nodes first. After that, grab Glyph sockets, then fill in Max Life and defensive nodes.
Dominate becomes especially strong later on and is a great place to dump extra points for more Overpower scaling.
Mercenary
How it Plays
While farming, gameplay is extremely simple.
Use
Sever and
Blood Mist to fly through the map, then drop
Blood Waves to instantly clear packs and keep moving.
Most supporting mechanics happen automatically through summons, runes and passives, so outside of mobility you'll rarely need to press anything.
Against tankier enemies, use
Bone Prison to gain attack speed, maintain Vulnerable and generate additional Essence.
For pushing, you can swap
Sever for
Corpse Tendrils to improve grouping, but the overall gameplay stays largely the same.
This build is all about staying mobile and turning Blood Wave into a one-button screen wipe.
How it Works
This build completely changes how
Blood Wave functions.
Hematolagnia Blood Wave removes Blood Wave's cooldown and turns it into a spammable Core Skill, meaning we no longer build around cooldown reduction or manually collecting Blood Orbs to enable damage.
Instead, the build focuses on solving resource and scaling Overpower.
Blood Wave constantly maintains Overpower stacks, allowing us to abuse some of the strongest damage multipliers in the game through Banished Lord's Talisman, tempers and charms.
Everything else exists to support this loop.
Summons generate Blood Orbs and corpses, runes automate curses and utility, and our supporting skills provide mobility, Vulnerable uptime and resource generation.
The end result is a build that mostly moves, casts Blood Wave, and lets the supporting systems do the rest.
