Gearing is very simple!

You should focus on getting gear that has LIFE, ARMOUR, % OF ARMOUR ALSO APPLIED TO ELEMENTAL DAMAGE & RESISTANCES. The separate important states will be covered below for each part.

MAIN WEAPON:
Get a Talisman with LOTS of Physical Damage and +to attack skill levels. The Best you can get.
A general rule ill follow: 650pDPS for map tier 1-13, 750 pDPS for tier 14-15 and 850+ pDPS for juiced tier 15 and 16s. Try fit in Leech if you can. Also a +Maximum rage variant is great but not needed.

AMULET:
Get one with +Melee skills and Anoint Tribal Fury

RINGS:
Best for Chaos resistances. Use these to max that. Try get Accuracy to increase the effect of Falcon Dive.

HELMET: Nothing special, just get life and resists.

BOOTS: 30%+ Movement speed

GLOVES: Get LIFE & MANA Leech! Its very important that you get these as its vital for our sustain.

BODY: Get a Ornate Plate with a max roll of the "Regenerate 2.5% of Maximum life per second" as we need this to maintain Berserk

BELT: Ryslatha's Coil is a beast for any physical build. But this is also not 100% needed. Feel free to swap it out for a Headhunter if you find one.

Skills and supports explained:

Our main skills are Shred, Pounce and Cross Slash which we rotate between in that order. See "HOW IT PLAYS" section for more detail on rotations.

Shred
Brutality III > Rapid Attacks II > Rage III > Armour Break III > Uruk's Smelting

This is our main trash mob clearer, our Armour breaker & most importantly, our Main Rage generator.
This with the Tribal Fury anointment will allow us to clear packs quick and efficiently all while generating the rage to spend on our other skills. It also feels sick to slash packs down!

Pounce
Brutality III > Bleed IV > Rageforged II > Impale > Cooldown Recovery II

This is best viewed as out PRIMING Skill. Which means it applies debuffs that we can exploit with Cross Slash. First is the mark it applies, this add 3% damage per one of your minions including yourself and will summon a wolf minion on marked targets death, which allows more future damage. Second is incisions are applied by both the pounce and the wolfs from Bleed IV, but neither can consume the incision to trigger the bleed, which allows us to do so with our other attacks. Lastly is Impale, which will add a % of our attacks to our next attack, also preventing pounce or the wolfs from consuming the Impale. Its also cool to jump around as a werewolf!

Cross Slash
Brutality III > Heft > Rageforged II > Deep Cuts II > Incision

This is our main BLEED Skill. Its scaling large bleed magnitudes with Deep Cuts II and Incision. We use this ONLY after we use Pounce as we want to maximise the bleed damage. This is the main boss killer.

Ferocious Roar
Prolonged Duration III > Tireless > Cooldown Recovery II > Efficiency II > Corrupting Cry II

This is how we generate Rage for our Pounce & Cross Slash during boss fights. You will wont to pop it at the start of a fight within a safe distance and then Pounce/ Cross Slash combo and repeat.

Herald of Blood
Bleed III > Poison III > Admixture > Bursting Plague > Deadly Poison II

This thing was introduced in 0.2 and is a GAME CHANGER for any bleed build. It makes all enemies explode if they have blood lose, which they always will! We are using Bursting Plague tech to make clear even more INSANE! Herald of Blood Explodes and applies Bursting Plague which then has its own explosion which can apply Bleed III which in turn Triggers Herald of Blood explosion. FUN STUFF!

Berserk
Vitality II

New this league along with the talismans. This allows us to increase the effect of our rage, thus giving us more damage as long as we are high rage. The down side is the 0.1% max life degeneration we get, BUT that's easily fixed by our passives and gear so we don't see any downsides. It is also the reason we want to maintain the uptime or our max rage.

THE TREE and Campaign buffs!

First lets talk Jewels:
You should have 3 jewel sockets by 90 and you will want to get the below.
%Increased global physical damage, Increased Melee Damage & Increased bleed duration

Ascendency order:
Colossal Capacity, Hulking Form, Mysterious Lineage, Crushing Impacts
(you can get Mysterious Lineage first if you want a lil extra tank while leveling)

Tree Order:

Please see above. I have listed the best order to obtain the nodes but please use your own feel with it. If you feel squishy, get more defence nodes first etc.

Campaign Buff choices:

Act 2: Increased Charm Effect in Valley of Titans

Act 3: Stun Threshold from Venom Crypts

Act 4: Choose the Attributes in Halls of the dead & Get +30% Increased Life recovery from flasks in Abandoned prison

Interlude: Get 15% Increased Global Defences in Qimah

How it Plays

The play style depends on whether you are fighting a single target or packs. But is very simple with small rotations.

Rotations:

Packs:
Shred will teleport you to a pack thats in range and will break their armour quickly. This will be enough to kill most packs, but if there is stragglers or rares then throw in a Pounce followed by a Cross Slash

Single target:
Use Ferocious Roar to generate rage then Pounce followed by a Cross Slash. If you can do it safely, you can throw in a few Shred hits to increase Armour break.
