1 - Fracture the +3 level of all melee skills. To do so, chaos spam 1 mod suffix rare amulet until you get it, then add 2 more mods with exalts, and a UNREVEALED abyss mod with collarbone, then fracture
2 - Essence of the Breach for +20% max quality. Use Reaver Catalyst until 34% quality. Annul back down to only the fracture
3 - Use a Omen of Sinistral Necromancy (prefix) + Altered Collarbone to try and get the Anaemia mod. If it fails, then remove with Annul and try again (cheaper than using omen of light, but won't guarantee you good prefix/suffixes after)
4 -Then you can suffix omen exalt, and remove with suffix omen perfect essence of enhancement
5 - Perfect exalt
Extra - If you want even better, you can first chaos spam before step 3, to get 45+ spirit. Then Omen of light with Altered Collarbone if you don't hit it instead of annulling
- 
Solemn Maul is sanctified for +6 level to all melee skills
1 - Solemn Maul or any high ilvl two-handed mace (81+) with a low strength requirment
2 - Buy or roll +5 to all melee skills (ideally it has Strength and life on kill suffixes, not mandatory)
3 - Fill prefixes/suffixes with exalts (mods don't matter)
4 - Perfect Essence of Abrasion for Gain % as extra phys damage (skip this step if you want)
5 - Omen of Sinistral Necromancy (prefix) + Omen of the Liege (Amanamu) + Jawbone and reveal % increased damage against enemies with fully broken armour. If this removes mod from step 4, repeat step 4-5
6 - Omen of Sanctification + Divine Orb and pray! If you don't get +6 to all melee skills then restart

Atziri's Splendour with as much energy shield as possible, make sure the 20% elemental resistance is still there as a mod. Helps if it has the no attribute requirement as well
- 
Prism of Belief with +3 to Fortifying Cry

Lavianga's Spirits with a 
Ultimate Mana Flask if you can't sustain mana
- You can runeforge 
Crown of the Victor and 
Saffell's Frame. You have to corrupt them after if you want additional sockets. You can buy 2 socket versions without a corruption first as well

Raven-Touched Shard for Crown of the Victor then I recommend 
Defensive Stance for 90% max resistance. Otherwise
