NOTE: Huge survivability boost from just upgrading your health flask whenever you can (lvl 10/16 etc)

Support gem 1: put Concentrated Effect on Mace Strike
support gem 2: put Impact Shockwave on Boneshatter
skill gem 2: get 
Shield Charge
support gem 3: Martial Tempo on Mace Strike
support gem 4: get Magnified Effect on Boneshatter
skill gem 3: get 
Infernal Cry
Spirit gem: get 
Herald of Ash
skill gem 4: get Perfect Strike (lvl 14) and have 2 supports ready to add Window of Opportunity & Martial Tempo
add Martial Tempo to every attack to make things smoother.
Once in act 3 add either Close Combat or Fire Infusion to perfect strike!
