Palm of the Dreamer isn't required. For clear setup u can use a Quarterstaff and Shattering Palm for explosive clear

Crown of the Pale King and Briarpatch are required core unique items for thorn. The best corruption is just as many sockets as possible

Rare items focus on Armour, Armour also apply to Elemental Damage and Life. Some flat Thorn damage roll is welcomed but not 100% required.

Rare Spear weapon: The only mod that scale Thorn dmg from Weapon is Extra damage as X damage, which we can guarantee from Perfect Essence of Abberasion (physical) in this case.

If you use a Quarterstaff instead of Spear and Palm of the Dreamer for clear setup with Shattering Palm, then as much flat dmg as possible is good. Onslaught mod from Perfect Essence of Haste is good for mapping, also cheap and guaranted

Megalomaniac with either Bleeding Immune or Corrupted blood Immune is required to AFK Simulacrum 3 all the way through. You get both Immunities from Megalomaniac and Annoint the other one on your Defiance of Destiny amulet
