Rampage: Demonic Smash to move faster (or play 
Command Fallen: Meat Shields for More push DPS)

Dark Prison: Chain Aura for to apply Weakness (20% Less damage taken) And for Fortification
If Out of mana make sure you copy build correct but your problem is likely
not using

If dying make sure to have 
Sadistic Aspect on Amulet via 
Kullean Tuning Prism
Where to Farm Warlock Items
Infernal Homunculus = Urivar
Elegy = Lord Zir 
Hand of Apotheosis = Bartuc
Moloch's Beating Flame = Butcher
The Eightfold Idol = Beast In Ice
Temerity = Craft in Cube or Harbinger of Hatred 
Fer of Abaddon's Flesh (All sets can be found anywhere)

Aspect of Fiendish Oppression Like any aspect is just Salvaging a Legendary Aspect
Stone of Jordan and 
Leoric's Crown can be found from Baal
(These CANNOT be crafted in cube via upgrade to Unique)
If Missing any any items Try using

Aspect of Scorching Heat
Brutal Aspect
Aspect of Ignition
You can use 
Wendigo Brand for More DPS over Stone of Jordan But you Will feel Squishy
Build is still playable without
