Grace of the Goddess, best weapon by far, because our shield is our main damage source, and getting sources of phys as extra elements is great!
Shield - Emperor's Vigilance the foulborn version. Try to get as much energy shield and armor on it as your budget allows.
Amulet -
Defiance of Destiny, best in slot, provides great resistances, and last line of text "gain X% of missing unreserved life before being hit by an enemy" combined with our rings and
Progenesis, will help us tank the biggest of hits.
Belt -
Arn's Anguish will convert your endurance charges to brutal charges, we have 11 endurance charges, converted to brutal charges, meaning we get 33% chance to deal triple damage.
Boots -
Ralakesh's Impatience (Endurance) the endurance version, making it so even though we convert our endurance charges to brutal charges, we still count as having max endurance charges, making us very tanky and able to scale the jugg ascendancy, while keeping the benefits of
Arn's Anguish.
Ring - A rare ring with +1 max endurance charge implicit, you buy the base, spam essences of Scorn until you hit T1 damage taken recouped as life, lock the suffixes, reforge chaos in hopes for high tier of chaos res. once suffixes are complete, bench craft mana, and exalt slam for high tier or life, then craft non-channeling cost.

Helmet -
Giantslayer Helmet would be the best in slot with fractured 5% additional physical damage reduction. This is a delve mod, meaning it's drop only, and can't be crafted. If the base is too expensive, buy a lower armor base, or you can move the unfractured physical damage reduction from one lower base to a proper base using recombinators, but IDK how that works exactly, so better do your own research. Once you got your fractured base, spam Hollow Fossils, until you either hit T1 accuracy, or an open suffix. If you hit an open suffix, lock the suffixes, and reforge attack, in hopes of getting T1 accuracy, then lock them again using Wild Bristle Matrons in the menagerie (they are cheaper than crafting suffixes cannot be changed) and slam veiled chaos orbs, until you unveil +2 AOE gems, craft % armor, and slam either with Exalted orbs, or Foulborn Exalted orbs until you hit a good tier of life. Third prefix can be whatever, bench craft armor or something.
Gloves -
Leviathan Gauntlets with fractured temple mod for 50% increased damage with hits against chilled enemies. Same as the boots, Hollow Fossils until open suffix, lock suffixes, reforge speed for T1 attack speed, lock suffixes with Wilk bristle Matron, veiled chaos until phys to lightning conversion or AOE, block %armor and slam exalts or foulborn exalts until good tier of life, bench craft the mod you didn't unveil, either phys to lightning convert or AOE.
Body Armor -
Royal Plate with fractured 10% reduced damage taken from crits is ideal, then spam dense fossils for your triple T1 prefixes, lock prefixes, reforge phys in hopes of hitting T1 additional phys damage reduction. If you get a base with Fractured 8% additional phys damage reduction, spam essences until you hit a good resist, finish prefixes with eldritch currency.

Oriath's End is great for clear and blowing up screens, combined with the flesh and flame from Chieftain grating you
Hinekora, Death's Fury, will have you speeding through maps
Cinderswallow Urn (Ignited enemies have Malediction), Malediction is great for tankyness and damage, all your damage will ignite and it will be up most of the time
Quicksilver Flask with either crit or reduced effect of curses during effect
Diamond Flask with either crit or reduced effect of curses during effect

Elegant Hubris (Caspiro Supreme Ostentation) socketed next to
Eternal Youth converting the keystone to Supreme Ostentation, making you able to disregard all attribute requirements. You want to get 10% chance to gain a frenzy charge on hit and 10% increased damage per endurance charge
Tecrod's Gaze with 3-4
Murderous Eye Jewel is a great combination, giving you a bunch of crit multi, accuracy, life and attack speed if you've dealt a critical strike recently
A watchers eye with precision attack speed and precision crit multi is great, 3rd mod would probably be something related to vitality, like increased life regeneration rate or something.
An Impossible Escape - Imbalanced Guard - will give you easy access to some accuracy and more maximum resistances
Other rare jewels should have triple crit multi (global, fire skills, elemental skills) and % increased maximum life
Also using 3 Large, 12 passive clusters, ideally with double 3% increased attack speed, one with phys skills, and one with fire skills. You get a base, fracture one of the attack speed mods, and use Refracting Fog to change the base of the jewel, use Reforge Speed at the harvest bench to get the 2nd attack speed mod, and try to slam for life and 35% increased effect.
